LATE HAEMOPHILLUS ENDOCARDITIS AFTER SEPTAL CLOSURE BY AMPLATZER

Abstract Introduction Atrial septal defects are the second most frequent congenital heart diseases and their treatment of choice is percutaneous approach. Complications associated infrequent mainly related with puncture. Other complications such as embolism or endocarditis uncommon. Clinical case A 17-year-old woman whose only medical history interest was an ostium secundum-type atrial defect that treated percutaneously implanting a 24-mm Amplatzer occlude. 15 months after procedure she came to emergency room presenting 40°C fever headache. Physical examination tests results were normal, being discharged antibiotic treatment. She returned 24 hours later due persistence same symptoms. An echocardiogram performed, revealing endocarditic growth uncomplete endothelization amplatzer. Following 10 days antibiogram-guided therapy good response, underwent surgery. The device extracted closed. PCR positive for H. Parainfluenzae. patient on 7th day therapy. Discussion This relevant late presentation IE, incomplete caused by IE germ source negative blood culture. Despite low prevalence these complications, it necessary rule them out through imaging well microbiological tests. Appropriate length bacterial coverage usually extends from 6 1 year implantation. Documented cases prosthetic incorrect endothelial can lead reassessing time period during which prophylaxis should be maintained.
